First Connections
First Connections supports moms by empowering them with the knowledge and resources needed to care for themselves and nurture their newborns.
Registered nurses and social workers provide culturally competent, comprehensive services in the client’s home. Services can include blood pressure screenings for moms, weight checks for infants, connections to the Department of Human Services, WIC, obstetricians, pediatricians, housing, and domestic violence shelters. All children in the home receive developmental screenings, and staff from Children’s Friend make referrals to local school departments as needed.
Who is Eligible?
- Birthing hospitals use a screening tool supported by the Department of Health to make referrals
- Mothers can self-refer by reaching out to the Department of Health
- Community providers, like obstetricians or pediatricians can make referrals
- DCYF can refer families
